Action for insurance denial - NO INDIVIDUAL PROVIDER ID # IN BOX 24K

Denial Reason: NO INDIVIDUAL PROVIDER ID # IN BOX 24K


Action:  

If a provider is enrolled as a group practice with a carrier, he would have both individual and group id# issued by the carrier.  The individual id# needs to be printed in slot 24k of HCFA and the group# in slot 33. We will get this rejection whenever the individual id# is not printed in slot 24k. Whenever you see this rejection, you may find that we have printed individual id# also in slot 33 instead of 24k.
